Thank you for choosing our practice for your dental care. We realize that every person’s financial situation is different. In an effort to keep overall costs down for our patients, we provide the following payment options; thus eliminating the need to generate thousands of billing statements, time spent tracking accounts, and postage for mailing statements.
Our payment options include the following:
1.Payment in full at the time of your visit: Cash, check, or credit card.
2.Major Service; two payment option: We offer a two payment option for crown, bridge, and denture treatment. We ask that you pay one-half of your co-payment at the first appointment and the second half at the delivery date appointment.
3.Credit card option: Allows you to make three equal monthly installments with your credit card. One third is due at the first appointment, one-third is due thirty days later, and the remaining one-third is due sixty days from the initial appointment. Our office personnel will charge these payments by processing your credit card on the due dates. Eligibility for this option will be determined on an individual basis.
4.Term Loan: We accept CareCredit. You must qualify prior to treatment for CareCredit to be eligible for this option, and can apply online at www.CareCredit.com. Click on the CareCredit logo for more information.
Our office will do everything possible to help you understand and make the most of your dental insurance benefits. We are happy to verify your insurance benefits and file all insurance claims for you. You are responsible for paying all charges not covered by your insurance company. The amount of coverage received will depend on the type of plan you or your employer purchased. Dental insurance is a contract between you and your insurance company. We encourage you to request a pre-authorization to your insurance company prior to treatment.
There will be a $35 service charge for all returned checks.
All delinquent accounts over 120 days are subject to collections and all related collection fees.
In order to be fair to all patients, we require a 48 hour notice to cancel or change your appointments. A $50 fee will be assessed for any short notice cancellation, short notice reschedule, or missed appointment.
